Yes, you can but not only in smtp_active_hostname also in helo_data directives, I explain it in this post

So, seems 10.0.0.223 is your primary ip and that is the reason exim is getting another ptr. I don’t want to mess your net conf so I suppose the “easy” way is to modify exim4 to use the right hostname. Remember to modify the PTR record of your public ip to point to this new hostname.
